Botulinum Toxin (Neurotoxin) Pre-Treatment Instructions

Avoid aspirin, ibuprofen (Advil/Motrin), naproxen (Aleve), fish oil, vitamin E, garlic supplements, ginkgo, St. John’s Wort, and other blood-thinning supplements/medications for 5–7 days prior to treatment when medically appropriate and approved by your prescribing provider.
Avoid alcohol for 24 hours before your appointment to help reduce bruising and swelling.
Arrive with a clean face free of heavy makeup if possible.
Avoid scheduling treatment immediately before major events, as mild redness, swelling, or bruising may occur.
Notify your provider if you:
Are pregnant or breastfeeding
Have a neuromuscular disorder
Have had prior facial surgery
Have active infection, rash, or irritation near the treatment area
Have experienced previous reactions to botulinum toxin products
Are currently taking antibiotics, muscle relaxers, or blood thinners
Patients prone to bruising may benefit from starting oral arnica several days before treatment if medically appropriate.
Stay well hydrated prior to treatment.
Please arrive on time to allow for consultation, photographs, and treatment planning.

Botulinum Toxin (Neurotoxin) Post - Treatment Instructions

Please review and adhere to the following post-treatment guidelines in preparation for your botulinum toxin appointment. The following restrictions are in place to minimize the risk of any potential complications

• It is best to wear no makeup or lipstick until the next day. Earlier use can cause infection.

• Do not raise your body temperature for 4 hours following treatment (no exercise, hot tubs, saunas, being out in the sun, etc.). It takes about 2-4 hours for the neurotoxin to bind to the nerve in order to start working and an increase in internal body temperature or sweating can make the neurotoxin treatment ineffective.

• Avoid touching or manipulating the treated areas for 4 hours. In addition, do not lie down within this timeframe. We want the neurotoxin to stay in the muscles that it was specifically placed to avoid poor outcomes like a dropped eyebrow or eyelid.

• Avoid massages, facials, chemical peels or other aesthetic treatments for at least 1 week after treatment.

• Avoid wearing hats, visors, headbands or anything on the head that has the potential to move the neurotoxin from the area it was injected.

• It will take about 24 hours for all of the injection sites to heal. To avoid infection, avoid wearing makeup on the day of the treatment. The face can be washed like normal at night (at least 4 hours post-treatment).

• Neurotoxins treatments typically start to take effect around 2-10 days, but everyone is different. Please give the neurotoxin a full 2 weeks to take effect before deciding if you need more. Don’t love it or hate it for 2 weeks!

• Be sure to schedule a 2-week follow-up appointment so your injector can assess your results and perform any enhancements if needed. It is not recommended to inject beyond 2 weeks post treatment, so please ensure that you keep this appointment.

• Neurotoxin appointments should be scheduled every 3-4 months to maintain the best results, if using Daxxify appointments should be scheduled every 6 - 9 months.

BOTULINUM TOXIN TYPE A INFORMED CONSENT

The purpose of this informed consent form is to provide written information regarding the treatment, contraindications, and risks of the procedure named above. This material serves as a supplement to the discussion you have with your RN injector. You will be receiving treatment from a provider practicing in collaboration with Nurtured by Nurses. In our ongoing efforts to provide you with the best possible service we ask that you carefully review this consent form and ask any questions necessary to help you fully understand it. THE TREATMENT

Botulinum toxin (Botox®, Xeomin®, Dysport®, Letybo®, Dysport®, Daxxify®) is a neurotoxin produced by the bacterium Clostridium A. Botulinum toxin can relax the muscles on areas of the face and neck which cause wrinkles associated with facial expressions or facial pain. Treatment with botulinum toxin can cause your facial expression lines or wrinkles to be less noticeable or essentially disappear. Areas most frequently treated are: a) glabellar area of frown lines, located between the eyes; b) crow’s feet (lateral areas of the eyes); c) forehead wrinkles; d) radial lip lines (smokers lines), e) head and neck muscles for treatment of headaches, migraines and trigger point therapy, f) palm of hands, armpits and feet for treatment of hyperhidrosis. Botox is diluted to a very controlled solution and when injected into the muscles with a very thin needle, it is almost painless. Patients may feel a slight burning sensation while the solution is being injected. The procedure takes about 15-‐ 20 minutes and the results can last up to 3 months, when using Daxxify results may last up to 6 months. With repeated treatments, the results may tend to last longer.

There is a difference in dosing when using Dysport or Daxxify as compared to the other neurotoxin agents. Dysport requires a 3:1 ratio compared to Botox, Xeomin or Letybo. Daxxify requires a 2:1 ratio compared to Botox, Xeomin or Letybo.

CONTRAINDICATIONS

I do not have any allergies to the toxin ingredients, human albumin, or other ingredients that may be used during this treatment, including but not limited to isopropyl alcohol, lidocaine, benzocaine, tetracaine or phenylephrine.
I do not have an active infection and/or am not currently on antibiotics (unless for prophylactic reasons only)
I am at least 18 years of age (or have a referral from my primary care provider or medical specialist)
I am not currently pregnant or breastfeeding (or have a referral from my primary care provider or medical specialist)
I have not been treated with neurotoxin within the past 3 months, excluding treatment within the past 14 days for which I am now being seen for follow up care

RISKS AND COMPLICATIONS

Before undergoing this procedure, understanding the risks is essential. No procedure is completely risk‐free. The following risks may occur, but there may be unforeseen risks and risks that are not included on this list. Some of these risks, if they occur, may necessitate hospitalization, and/or extended outpatient therapy to permit adequate treatment. It has been explained to me that there are certain inherent and potential risks and side effects in any invasive procedure and in this specific instance such risks include but are not limited to:

Post treatment discomfort, swelling, redness, and bruising,
Double vision,
A weakened tear duct,
Post treatment bacterial, and/or fungal infection requiring further treatment,
Allergic reaction,
Minor temporary droop of eyelid(s) in approximately 2% of injections, this usually lasts 2-3 weeks,
Occasional numbness of the forehead lasting up to 2-3 weeks,
Transient headache and
Flu-like symptoms may occur.

PREGNANCY & OTHER MEDICAL CONDITIONS

I am not aware that I am pregnant. I am not trying to get pregnant. I am not lactating (nursing). If pregnant or lactating, I understand I must provide Nurtured by Nurses with a medical clearance from my primary care provider or OB/GYN for the specific treatment I am receiving.

I do not have or have not had any major illnesses which would prohibit me from receiving the services I am requesting. I certify that I do not have multiple allergies or high sensitivity to medications, including but not limited to lidocaine.

I do not have any significant neurologic disease including but not limited to: myasthenia gravis, multiple sclerosis, Lambert-Eaton syndrome, amyotrophic lateral sclerosis (ALS), and/or Parkinson’s. Or, if I do have a neurologic disease or any other medical condition, I have discussed with my primary medical provider or specialist and have determined I am safe to proceed with this treatment. I have fully disclosed all medical conditions during my exam with the Nurtured by Nurses provider. I understand that receiving optional services during an exacerbation of any medical condition is not advised and that I should reschedule my appointment if experiencing any exacerbation of a chronic condition or any acute illness.

I understand that any chronic medical condition or acute illness may place me at greater risk for complications and I will not hold the nurse injector or Nurtured by Nurses liable for any complications that are a result of my medical conditions.

PAYMENT

I understand that this is an "elective” procedure and that payment is my responsibility and is expected at the time of treatment.

RIGHT TO DISCONTINUE TREATMENT

I understand that I have the right to discontinue treatment at any time. Premature discontinuation of treatment may lead to suboptimal outcomes or unintended aesthetic effects.

Results

I am aware that when small amounts of purified botulinum toxin are injected into a muscle it causes weakness or paralysis of that muscle. This appears in 2 – 10 days and usually lasts up to 3 months but can be shorter or longer. In a very small number of individuals, the injection does not work as satisfactorily or for as long as usual and there are some individuals who do not respond at all. I understand that I will not be able to use the muscles injected as before while the injection is effective but that this will reverse after a period of months at which time re-treatment is appropriate. There is the possibility of a poor or inadequate response from any treatment. Each individual responds differently. Full results may take several weeks to appear. No guarantees about results have been made. Failure to follow aftercare instructions may lead to suboptimal outcomes or unintended aesthetic effects. There are no refunds available for unsatisfactory results.
